Abstract

PURPOSE

The purpose of the present investigation was to locate the instantaneous rotation center of the mandible during maxillary surgical impaction, and explore the relationship between automatic rotation center of the mandible and maxillary elevation amount, the length of the mandible and mandibular plane angle.

METHODS

Twenty-five patients who underwent maxillary Le Fort I impaction without concomitant major mandibular ramus split osteotomies were included. The preoperative and postoperative lateral cephalograms were used to evaluate the surgical changes and locate the mandibular autorotation center with Reuleaux method. The automatic rotation center of the mandible was compared to the maxillary elevation amount, the length of the mandible and mandibular plane angle with Pearson correlation and linear regression analysis. The data was analyzed by SPSS 13.0 software package.

RESULTS

The mandibular automatic rotation center was located in average 15.64 mm below and 0.82 mm behind the center of the condylar head in these 25 patients. The correlation analysis demonstrated a positive correlation between maxillary elevation amount, the length of the mandible and the position of the rotation center of the mandible. Similar positive correlation was presented between the mandibular plane angle and the vertical position of the rotation center of the mandible.

CONCLUSIONS

The rotation center in 25 cases were located outside the condylar head. The maxillary elevation amount, the length of the mandible and the mandibular plane angle was positively correlated to the position of the rotation center of the mandible.

目的

本研究的目的是确定上颌骨手术牵张成骨过程中下颌骨的瞬时旋转中心,并探讨下颌骨自动旋转中心与上颌骨抬高量、下颌骨长度和下颌平面角之间的关系。

方法

纳入25例接受上颌Le Fort I型牵张成骨且未同时进行下颌升支劈开截骨术的患者。术前和术后的头颅侧位片用于评估手术变化,并采用勒洛三角形法确定下颌骨自动旋转中心。通过Pearson相关性分析和线性回归分析,将下颌骨自动旋转中心与上颌骨抬高量、下颌骨长度和下颌平面角进行比较。数据采用SPSS 13.0软件包进行分析。

结果

这25例患者的下颌骨自动旋转中心平均位于髁突头中心下方15.64 mm和后方0.82 mm处。相关性分析表明,上颌骨抬高量、下颌骨长度与下颌骨旋转中心位置呈正相关。下颌平面角与下颌骨旋转中心的垂直位置之间也呈现类似的正相关。

结论

25例患者的旋转中心位于髁突头之外。上颌骨抬高量、下颌骨长度和下颌平面角与下颌骨旋转中心位置呈正相关。
