The aim of this cross-sectional study is to compare the local and systemic levels of soluble receptor activator of nuclear factor-κB ligand (sRANKL), osteoprotegerin (OPG), a proliferation-inducing ligand (APRIL), B-cell activating factor (BAFF), interleukin (IL)-6, and IL-8 in biofluids of patients with thalassemia major (TM) with or without gingivitis.
Seventy-seven patients are included in this study (TM, n = 29; systemically healthy, n = 48). Gingival crevicular fluid (GCF), saliva, and serum levels of IL-6, IL-8, sRANKL, OPG, BAFF, and APRIL were determined by enzyme-linked immunosorbent assay. Data were analyzed by appropriate non-parametric or parametric statistical tests.
Median GCF, serum, and saliva levels for BAFF (P <0.001) and IL-6 and IL-8 (P <0.005) were higher in TM gingivitis than in systemically healthy gingivitis (P <0.001). GCF, serum, and saliva levels for APRIL, sRANKL, IL-6, and IL-8 were higher in TM than in systemically and periodontally healthy comparison groups (P <0.05). Positive correlations were found between bleeding on probing (BOP), plaque index (PI) scores, and GCF APRIL, serum sRANKL, serum OPG, and sRANKL concentrations in TM groups (P <0.05). Several significant positive correlations were found between BOP, PI scores, and biofluid parameters also in systemically healthy groups.
TM may have a role in the underlying systemic hematologic condition and potentially affect gingival inflammation via dysregulation of lymphocytes and increased activation of osteoclasts.
本横断面研究旨在比较重度地中海贫血(TM)患者伴或不伴牙龈炎时生物流体中核因子κB受体活化因子配体(sRANKL)、骨保护素(OPG)、增殖诱导配体(APRIL)、B细胞活化因子(BAFF)、白细胞介素(IL)-6和IL-8的局部和全身水平。
本研究纳入77例患者(TM患者,n = 29;全身健康者,n = 48)。采用酶联免疫吸附测定法测定龈沟液(GCF)、唾液和血清中IL-6、IL-8、sRANKL、OPG、BAFF和APRIL的水平。通过适当的非参数或参数统计检验分析数据。
TM牙龈炎患者的BAFF(P <0.001)、IL-6和IL-8(P <0.005)的龈沟液、血清和唾液水平中位数高于全身健康的牙龈炎患者(P <0.001)。TM患者的APRIL、sRANKL、IL-6和IL-8的龈沟液、血清和唾液水平高于全身和牙周健康的对照组(P <0.05)。在TM组中,探诊出血(BOP)、菌斑指数(PI)评分与龈沟液APRIL、血清sRANKL、血清OPG和sRANKL浓度之间存在正相关(P <0.05)。在全身健康组中,BOP、PI评分与生物流体参数之间也发现了几个显著的正相关。
TM可能在潜在的全身血液系统疾病中起作用,并可能通过淋巴细胞失调和破骨细胞活化增加影响牙龈炎症。
