The author describes specific features of the artery wall structure appearing with the decreasing caliber of the vessel. Special grounds for certain elements of the operative technique in the surgery of microvessels are given on the basis of the assessment of results of 300 microanastomoses in experiment and 500 microvascular anastomoses made in the clinic. It was found that the performance of correct anastomoses can be controlled more reliably when using the method of a step-by-step connection beginning from the most remote wall.
