Abstract

INTRODUCTION

Posterior dislocation of the hip joint with associated acetabular and intertrochanteric fracture is a complex injury. Early recognition, prompt and stable reduction is needed of successful outcome.

CASE REPORT

45 year old male patient presented with posterior dislocation of the hip with transverse fracture with posterior wall fracture of acetabulam and intertrochanteric fracture on the ipsilateral side. The complex fracture geometry was confirmed by CT scan. The patient was successfully managed by open reduction and internal fixation of intertrochanteric fracture was achieved with dynamic hip screw (DHS) plate fixation followed by fixation of acetabular fracture with reconstruction plate.

CONCLUSION

Hip dislocation combined with acetabular fracture is an uncommon injury; this article presents a unique case of posterior wall and transverse fractures of ipsilateral acetabulum with intertrochanteric fracture in a patient who sustained traumatic posterior hip dislocation. Early surgical intervention is important for satisfactory outcomes of such complex fracture-dislocation injuries.

髋关节后脱位合并髋臼及转子间骨折是一种复杂损伤。早期识别、及时且稳定的复位对于取得成功预后是必要的。

病例报告

一名45岁男性患者,因髋关节后脱位伴髋臼后壁横行骨折及同侧转子间骨折就诊。CT扫描证实了复杂的骨折形态。患者通过切开复位成功治疗,转子间骨折采用动力髋螺钉(DHS)钢板固定,随后髋臼骨折用重建钢板固定。

结论

髋关节脱位合并髋臼骨折是一种罕见损伤;本文呈现了一例因创伤性髋关节后脱位导致同侧髋臼后壁及横行骨折合并转子间骨折的独特病例。对于此类复杂骨折脱位损伤,早期手术干预对于获得满意预后很重要。
