An unexpected anatomopathological diagnosis--an adenomatoid neoplasm of the epididymis in a patient operated for hydrocele and suspected left chronic epididymis is described. After having briefly described the pathological anatomy and epidemiology of the adenomatoid neoplasm, the paper discusses the importance of a careful anamnestic and clinical assessment of commonly encountered diseases which are often considered clinically less important.
