A 22-year-old man with patent ductus arteriosus with severe pulmonary hypertension was operated upon successfully. The operation, trans-aortic direct closure of ductus, was done using temporary long external bypass. Postoperative course was uneventful, and the patient is now well 6 years and 8 months after operation. Postoperative cardiac catheterization revealed decreased pressure of pulmonary artery from 116 to 44 mmHg. The details of the operative procedure and postoperative course were described and a brief review of literatures was also done.
