The purpose of the present study was to assess the clinical effects of anti-microbial photodynamic therapy (PDT) after closed surface scaling in the treatment of peri-implant diseases. Ten patients with a total of 15 pairs of dental implants, showing clinical and radiographic signs of peri-implant diseases, were included in this study. In each patient, one implant randomly served as control implant and the other served as test implant. The control implants were treated with closed surface scaling only and the test implants received additionally PDT, using light with a wavelength of 630 nm and intensity of 2000 mw/cm for 120 seconds after application of photosensitizer in peri-implant sulcus. Clinical parameters were evaluated before and 1.5 and 3 months after treatment. Statistical analysis showed significant differences in probing pocket depth (PPD), clinical attachment loss (CAL), bleeding on probing (BOP), and gingival index (GI) at each time point between the two groups. There were no statistically significant changes with respect to any of the parameters in the control group. Complete resolution of BOP at 3 months was achieved in 100% of test implants. At 1.5 and 3 months, there were significant differences in the mean probing depth and CAL gain measurements at implants in the test group. The present study revealed that adjunctive use of PDT following closed surface scaling could lead to clinical improvement of peri-implant diseases. Further studies are necessary to confirm our results.
本研究的目的是评估封闭表面洁治后抗菌光动力疗法(PDT)在治疗种植体周围疾病中的临床效果。本研究纳入了10例患者,共15对牙种植体,这些种植体表现出种植体周围疾病的临床和影像学特征。在每位患者中,一个种植体随机作为对照种植体,另一个作为试验种植体。对照种植体仅接受封闭表面洁治,试验种植体在种植体周围龈沟应用光敏剂后,额外接受波长为630 nm、强度为2000 mw/cm²的光照射120秒的PDT治疗。在治疗前以及治疗后1.5个月和3个月评估临床参数。统计分析显示,两组在每个时间点的探诊深度(PPD)、临床附着丧失(CAL)、探诊出血(BOP)和牙龈指数(GI)存在显著差异。对照组的任何参数均无统计学上的显著变化。100%的试验种植体在3个月时BOP完全消退。在1.5个月和3个月时,试验组种植体的平均探诊深度和CAL增加量测量值存在显著差异。本研究表明,封闭表面洁治后辅助使用PDT可使种植体周围疾病得到临床改善。需要进一步研究来证实我们的结果。
