A case of autotransplantation of an intra-abdominal testis into the scrotum in a 9-year-old boy using a microsurgical technique without venous anastomosis is described in detail. The operation failed subsequently due to venous outflow obstruction and the testis underwent atrophy 6 months postoperatively. It was suggested that both the arterial and venous anastomoses may be necessary for the management of this difficult surgical problem.
