Abstract

To predict the prognosis of acute lowtone sensorineural hearing loss(ALHL)by using the videonystagmography(VNG) and electrocochleography(ECochG).A retrospective study was done in 70 patients with ALHL after long-term follow-up. The recurrence rate and rate of progression to definite Meniere 's disease were determined according to the results of VNG and ECochG tests at the onset of the first episode of hearing loss.Twentynine patients(41.4%)experienced recurrent hearing loss and 5 patients(7.1%)developed to Meniere's disease. In patients with an elevated SP/AP ratio and spontaneous nystagmus, the recurrence rate was 81.8%. However, in those with a normal SP/AP ratio and without spontaneous nystagmus, the recurrence rate was 23.3%.There was a statistically significant difference in the progression rate of Meniere's disease between the groups with(15.2%) and without spontaneous nystagmus.It has a certain reference value to predict prognosiswhen spontaneous nystagmus and an elevated SP/AP ratio are detected on initial examination in patients with ALHL.

通过使用视频眼震图(VNG)和耳蜗电图(ECochG)来预测急性低频感音神经性听力损失(ALHL)的预后。对70例ALHL患者进行了长期随访的回顾性研究。根据首次听力损失发作时VNG和ECochG测试结果确定复发率和进展为明确梅尼埃病的发生率。29例患者(41.4%)出现听力损失复发,5例患者(7.1%)发展为梅尼埃病。SP/AP比值升高且有自发性眼震的患者复发率为81.8%。然而,SP/AP比值正常且无自发性眼震的患者复发率为23.3%。有自发性眼震组(15.2%)和无自发性眼震组之间梅尼埃病的进展率存在统计学显著差异。在ALHL患者初次检查时检测到自发性眼震和SP/AP比值升高时,对预测预后有一定参考价值。
