The problem-oriented medical record: modification and simplification for rehabilitation medicine.

A modified POMR has been formulated for use in rehabilitation medicine. It incorporates the concept of the Weed system, modifies rehabilitation problems into seven functional categories, and modifies the problem-oriented progress note. It uses a problem-oriented team conference and a functional status form. These modifications, I believe, improve patient care, medical education, and facilitate the medical audit.
