To evaluate the efficacy of imiquimod 5% in periungual wart treatment.
A group of 40 patients were recruited to apply imiquimod 5 % cream once daily for 5 consecutive days per week in 8 weeks. They were classified into 3 levels: Mild (the total lesion area ≤ 25 mm), moderate (25 mm <total lesion area ≤ 50 mm), severe (total lesion area > 50 mm). The outcome was evaluated at the 4 and the 8 week. The result was graded as excellent (complete clearance), good (≥ 50% clearance) and poor (< 50% clearance).
The total area of the wart lesion got decreased significantly from the beginning to the 4 and the 8 week (36.7 mm vs 16.8 mm, p = 0.0001 and 16.8 mm vs 8.8 mm, p = 0.01). The complete clearance rate at the 4th week was lower than that at the 8th week significantly (22.5% vs 72.5%, p = 0.04). The clearance rate of patients suffering severe warts was lower significantly than that of mild/moderate patients (82.8% vs 45.5%, p = 0.03). The duration of the disease in people who responded completely to imiquimod was shorter than that of patients partially responded (10.2 ± 14.1 months vs 22.3 ± 14.3 months, p = 0.02). Adverse effects were not common, mild and local only. Recurrence rate after 6 months of follow up was 3.5%.
In conclusion, Imiquimod 5% cream is a safe and effective drug in the treatment of periungual warts.
评估5%咪喹莫特治疗甲周疣的疗效。
招募40例患者，每周连续5天每天外用一次5%咪喹莫特乳膏，共8周。将患者分为3级：轻度（总皮损面积≤25mm）、中度（25mm＜总皮损面积≤50mm）、重度（总皮损面积＞50mm）。在第4周和第8周评估疗效。结果分为优（完全清除）、良（清除率≥50%）和差（清除率＜50%）。
从开始到第4周和第8周，疣体病变总面积显著减小（36.7mm对16.8mm，p = 0.0001；16.8mm对8.8mm，p = 0.01）。第4周的完全清除率显著低于第8周（22.5%对72.5%，p = 0.04）。重度疣体患者的清除率显著低于轻度/中度患者（82.8%对45.5%，p = 0.03）。对咪喹莫特完全反应的患者病程短于部分反应的患者（10.2±14.1个月对22.3±14.3个月，p = 0.02）。不良反应不常见，仅为轻度且局限于局部。随访6个月后的复发率为3.5%。
总之，5%咪喹莫特乳膏是治疗甲周疣的一种安全有效的药物。
