A patient is reported who developed a brain abscess due to Listeria monocytogenes while on long-term steroid therapy and who was successfully treated medically without surgical intervention. The literature on listeria brain abscess is reviewed, and the importance of early recognition of this infection in any immunocompromised patient who develops focal cerebral dysfunction is emphasised.
