Abstract

OBJECTIVE

To evaluate the optic disc microvasculature in optic nerve head drusen (ONHD) vasculature in comparison to acute nonarteritic anterior ischemic optic neuropathy (NAION) and normal eyes using optical coherence tomography angiography (OCT-A).

METHODS

Ten eyes with ONHD, 10 eyes with acute NAION, and 10 healthy eyes were included in this prospective, comparative, observational case series. OCT-A imaging was performed on the optic discs. Qualitative grading was performed for dilation and tortuosity of the peripapillary vessels by 3 graders. Quantitative comparison was performed for peripapillary and inside disc vessel densities in nerve head (NH) and retinal peripapillary capillary (RPC) slabs.

RESULTS

The intergrader agreement for dilation and tortuosity of the peripapillary vessels was poor (0.313 and 0.182 for vascular dilation in nerve head and radial peripapillary capillary enface images, respectively, and 0.478 and 0.490 for vascular tortuosity in nerve head and radial peripapillary capillary enface images, respectively). In NH en face images, the vessel density measurements were statistically significantly different between the 3 groups (all p < 0.05). In RPC en face images, the vessel density measurements were statistically significantly different between the 3 groups (all p < 0.05) except for nasal peripapillary sector (0.08).

CONCLUSION

Despite poor intergrader agreement in qualitative analysis, quantitative OCT-A evaluation may differentiate optic disc edema due to NAION from pseudodisc edema due to ONHD.

目的

通过光相干断层扫描血管造影术(OCT-A)评估视神经乳头玻璃膜疣(ONHD)血管与急性非动脉炎性前部缺血性视神经病变(NAION)和正常眼的视盘微血管。

方法

本前瞻性、对照、观察性病例系列研究纳入了 10 只 ONHD 眼、10 只急性 NAION 眼和 10 只健康眼。对视盘进行 OCT-A 成像。3 名分级员对视神经头(NH)和视网膜神经纤维层毛细血管(RPC)平面的周边血管扩张和迂曲进行定性分级。对神经头和视网膜神经纤维层毛细血管平面的周边和内部盘血管密度进行定量比较。

结果

周边血管扩张和迂曲的分级员间一致性较差(NH 平面的血管扩张分别为 0.313 和 0.182,视网膜神经纤维层毛细血管平面分别为 0.478 和 0.490;NH 平面的血管迂曲分别为 0.313 和 0.182,视网膜神经纤维层毛细血管平面分别为 0.478 和 0.490)。在 NH 平面图像中,3 组间血管密度测量值差异均有统计学意义(均 P < 0.05)。在 RPC 平面图像中,3 组间血管密度测量值差异均有统计学意义(均 P < 0.05),除了鼻侧视盘扇形区(0.08)。

结论

尽管定性分析的分级员间一致性较差,但定量 OCT-A 评估可能有助于区分 NAION 引起的视盘水肿与 ONHD 引起的假性视盘水肿。
