Peripheral resistance to thyroid hormones is a type of unresponsiveness of the peripheral cells or tissues to FT3 and/or FT4. Generalised resistance to thyroid hormones affects the pituitary gland and most of the peripheral tissues. Selective pituitary resistance or central resistance to thyroid hormones include unresponsiveness of the pituitary gland, but the peripheral tissues are responsive. Selective peripheral resistance involves peripheral tissue or cellular resistance to thyroid hormones, but the pituitary gland is responsive.
We present a rare case of a female patient with partial peripheral resistance to thyroid hormones due to a point mutation coding for the beta subunit of the integrin molecule α(V)β(3) and concomitant Hashimotos thyroiditis. Clinically, the patients symptoms were in favour of hypothyroidism, and the laboratory results were in favour of the secondary hyperthyroid state. PCR protein amplification detected a point mutation coding for the membrane receptor, which mediates a signal via the MAPK pathway when bonded with thyroid hormones.
Peripheral resistance to thyroid hormones is a very rare condition and can often be misdiagnosed due to the broad spectrum of clinically similar differential diagnostic entities. Molecular analysis is required to confirm the exact underlying cause for the impaired peripheral sensitivity to thyroid hormones syndrome.
甲状腺激素外周抵抗是外周细胞或组织对FT3和/或FT4的一种无反应状态。全身性甲状腺激素抵抗会影响垂体和大多数外周组织。选择性垂体抵抗或中枢性甲状腺激素抵抗包括垂体无反应,但外周组织有反应。选择性外周抵抗涉及外周组织或细胞对甲状腺激素的抵抗,但垂体有反应。
我们报告了一例罕见的女性患者,因整合素分子α(V)β(3)β亚基编码的点突变以及合并桥本甲状腺炎而出现部分外周甲状腺激素抵抗。临床上,患者的症状提示甲状腺功能减退,而实验室检查结果提示继发性甲状腺功能亢进状态。PCR蛋白扩增检测到编码膜受体的点突变,该膜受体与甲状腺激素结合时通过MAPK途径介导信号。
甲状腺激素外周抵抗是一种非常罕见的疾病,由于临床鉴别诊断实体范围广泛,常易被误诊。需要进行分子分析以确认甲状腺激素外周敏感性受损综合征的确切潜在病因。
