Abstract

: To investigate the perioperative peripheral blood levels of CD4CD25 regulatory T cells, programed cell death 1 (PD-1), and lymphocyte activation gene 3 (LAG-3) in patients with advanced Siewert type II adenocarcinoma of esophagogastric junction (AEG). : Patients ( = 102) with advanced Siewert type II AEG underwent open total gastrectomy/proximal gastrectomy with a transhiatal resection of the distal esophagus and lymphadenectomy of the lower mediastinum and the abdominal D2 compartment. Flow cytometry was used to detect CD4CD25 T cells, PD-1 and LAG-3 expression on both CD4 and CD8 T cells in the peripheral blood of the Siewert type II AEG patients prior to surgery and on postoperative day (POD) 1, 3, 7, and 9. : The proportion of CD4CD25 T cells rapidly decreased on POD 1, then gradually increased and peaked at POD 7. The proportion of CD4PD-1 T cells significantly increased after surgery, reaching a maximum on POD 1, and remained significantly elevated on POD 3 compared to the preoperative day. The proportion of CD8 PD-1 and CD4LAG-3 T cells gradually increased after surgery and reached a peak at POD 7. The change in proportion of CD8LAG-3 T cells in the peripheral venous blood lymphocytes after surgery was not statistically significant. : The change in the CD4PD-1 T lymphocyte ratio may likely reflect the cellular immunity status of the perioperative period.

目的

研究进展期食管胃交界部(AEG)SiewertⅡ型腺癌患者围手术期外周血中CD4CD25调节性T细胞、程序性细胞死亡蛋白1(PD-1)和淋巴细胞活化基因3(LAG-3)的水平。

方法

102例进展期SiewertⅡ型AEG患者接受了开放全胃切除术/近端胃切除术,同时经裂孔切除远端食管,并清扫下纵隔和腹部D2区域淋巴结。采用流式细胞术检测SiewertⅡ型AEG患者术前及术后第1、3、7和9天外周血中CD4CD25 T细胞、PD-1和LAG-3在CD4和CD8 T细胞上的表达。

结果

CD4CD25 T细胞比例在术后第1天迅速下降,然后逐渐上升,并在术后第7天达到峰值。CD4PD-1 T细胞比例术后显著增加,在术后第1天达到最高,与术前相比,术后第3天仍显著升高。CD8 PD-1和CD4LAG-3 T细胞比例术后逐渐增加,并在术后第7天达到峰值。术后外周静脉血淋巴细胞中CD8LAG-3 T细胞比例的变化无统计学意义。

结论
