Few studies had assessed the auditory and preverbal skills of very young cochlear implant (CI) children with additional disabilities (AD) over a long period, especially in China. The aim of this study was to compare the early auditory and preverbal developmental trajectories in CI children with and without AD. The LittlEARS Auditory Questionnaire (LEAQ) was employed. 29 typically developing (TD) children and 17 with AD were involved (age at implantation less than 2 years). All children showed significant improvement in total LEAQ scores with CI use. Children with cerebral palsy (CP), developmental delay (DD) and white matter lesions (WML) scored lower than TD children since 3 months of CI use; a decreasing trend was observed from 24, 18 and 18 months of CI use, respectively. Children with higher nonverbal developmental quotients exhibited superior early auditory and preverbal skills. The development of early auditory and preverbal skills among CI-using children progressed more slowly in those with AD (CP, DD or WML) than in TD children, but the differences between the two groups gradually diminished over time. Nonverbal cognitive status has a positive effect on early auditory and preverbal abilities.
很少有研究长期评估患有其他残疾(AD)的低龄人工耳蜗(CI)儿童的听觉和言语前技能,尤其是在中国。本研究的目的是比较有和没有AD的CI儿童早期听觉和言语前的发育轨迹。采用了小儿听力问卷(LEAQ)。纳入了29名发育正常(TD)儿童和17名患有AD的儿童(植入时年龄小于2岁)。所有儿童在使用CI后LEAQ总分均有显著提高。自使用CI 3个月起,患有脑瘫(CP)、发育迟缓(DD)和白质病变(WML)的儿童得分低于TD儿童;分别在使用CI 24、18和18个月时观察到下降趋势。非言语发育商较高的儿童表现出更好的早期听觉和言语前技能。使用CI的儿童中,患有AD(CP、DD或WML)的儿童早期听觉和言语前技能的发展比TD儿童进展更缓慢,但两组之间的差异随着时间的推移逐渐减小。非言语认知状态对早期听觉和言语前能力有积极影响。
