Hip surgery in the presence of an aortofemoral bypass graft is dangerous. The use of an anterior spike retractor and the position of the leg in extreme adduction and rotation pose a potential threat to the graft. If hip surgery is necessary, a large trochanteric osteotomy should be performed to avoid both of these dangers.
