Cardiovascular disease (CVD) is the leading cause of death during pregnancy. Therefore, its detection early in pregnancy is of great importance. In this study, we aimed to investigate the rate and spectrum of CVD among healthy women in the first trimester of pregnancy.
A total of 900 women in the first trimester of pregnancy, who attended the antenatal outpatient clinic of a tertiary care center in Ankara, Turkey, for a routine pregnancy examination, were recruited into this prospective study. Patients with a history of chronic systemic diseases, CVD, and/or a family history of an early onset CVD, and multiple pregnancies were excluded. Patients who were included in the study underwent electrocardiography and transthoracic echocardiography by the same cardiologist.
The rate of newly diagnosed CVD cases among asymptomatic pregnant women without any cardiac or clinical risk factors was 5.2% (n=47). The following CVDs were detected: rheumatic heart disease (n=26; 55.3%), congenital heart disease (n=13; 27.6%), and isolated valve disease (moderate and severe) (n=8; 17%). The mitral valve prolapse was detected in 32 women (3.5%) and atrial septal aneurysm in 51 women (5.67%). Thirty women (3.3%) had arrhythmia.
The CVD rate of 5.2% among healthy women in the first trimester of pregnancy shows that the clinicians must keep in mind that during pregnancy, physiological changes in the cardiovascular system may aggravate an undiagnosed disease, and they should be alert even in case of mild cardiac symptoms that may interfere with pregnancy complaints.
心血管疾病(CVD)是孕期死亡的主要原因。因此,在孕期早期对其进行检测至关重要。在本研究中,我们旨在调查妊娠早期健康女性中CVD的发生率和类型。
共有900名妊娠早期女性参与了这项前瞻性研究,她们前往土耳其安卡拉一家三级医疗中心的产前门诊进行常规妊娠检查。排除有慢性全身性疾病、CVD病史和/或CVD早发家族史以及多胎妊娠的患者。纳入研究的患者由同一位心脏病专家进行心电图和经胸超声心动图检查。
在无任何心脏或临床危险因素的无症状孕妇中,新诊断出的CVD病例发生率为5.2%(n = 47)。检测到以下几种CVD:风湿性心脏病(n = 26;55.3%)、先天性心脏病(n = 13;27.6%)和孤立性瓣膜病(中重度)(n = 8;17%)。32名女性(3.5%)检测到二尖瓣脱垂,51名女性(5.67%)检测到房间隔瘤。30名女性(3.3%)有心律失常。
妊娠早期健康女性中CVD发生率为5.2%,这表明临床医生必须牢记,孕期心血管系统的生理变化可能会使未确诊的疾病加重,即使是可能干扰妊娠主诉的轻微心脏症状,他们也应保持警惕。
