Renal potassium excretion as the factor mainly responsible for potassium balance is reviewed. Clinical syndromes related to potassium wasting, abnormal distribution between intra- and extracellular space and potassium retention are outlined with reference to the pathophysiologic mechanisms. Some therapeutic considerations are presented.
