Vulvar lichen planus (LP) and vulvar lichen sclerosus (LS) are chronic inflammatory diseases that affect women's sexual health. In this study, our aim was to investigate sexual function, anxiety level and genital self-image in vulvar LP and vulvar LS patients.
This study was conducted on a total of 178 women who presented to the gynecology clinic between February 2019 and January 2020. The patients were divided into the following groups: group 1, vulvar LP ( = 21); group 2, vulvar LS ( = 59); group 3, fungal vulvitis controls ( = 48); and group 4, healthy controls ( = 50). The validated Female Sexual Function Index (FSFI), Beck Anxiety Index (BAI), and Female Genital Self-Image Scale (FGSIS) questionnaires were assessed in all women.
There were no significant differences among the groups with respect to age, parity, menopausal status, body mass index (BMI), vaginal intercourse past 1 month, marital status or educational status ( > .05). There were statistically significant differences between the vulvar LP and vulvar LS groups compared to control groups in terms of FSFI total scores and subscores ( < .001). When FGSIS and BAI scores were analyzed, significant statistical differences were found among the study groups ( < .001). A positive correlation was found between the FSFI and FGSIS scores in patients with vulvar LP and LS. Additionally, a negative correlation was found between the FSFI and BAI scores in patients with vulvar LP and LS.
Our study showed that sexual function, genital self-image and anxiety level were associated in vulvar LP and vulvar LS patients. Vulvar LP and vulvar LS patients with worse genital self-image have more sexual problems and anxious conditions.
外阴硬化性苔藓(LS)和外阴扁平苔藓(LP)是两种慢性炎症性疾病,会影响女性的生殖健康。本研究旨在调查 LP 和 LS 患者的性功能、焦虑水平和生殖器自我形象。
本研究共纳入 178 名 2019 年 2 月至 2020 年 1 月期间在妇科就诊的女性患者。患者分为以下组:组 1,外阴 LP(21 例);组 2,外阴 LS(59 例);组 3,真菌性外阴炎对照组(48 例);组 4,健康对照组(50 例)。所有女性均接受了经过验证的女性性功能指数(FSFI)、贝克焦虑量表(BAI)和女性生殖器自我形象量表(FGSIS)问卷评估。
各组间年龄、产次、绝经状态、体重指数(BMI)、过去 1 个月阴道性交、婚姻状况或教育程度无显著差异(>.05)。与对照组相比,外阴 LP 和 LS 组的 FSFI 总分和各分项评分存在显著差异(<.001)。当分析 FGSIS 和 BAI 评分时,研究组间存在显著统计学差异(<.001)。外阴 LP 和 LS 患者的 FSFI 与 FGSIS 评分之间存在正相关。此外,外阴 LP 和 LS 患者的 FSFI 与 BAI 评分之间存在负相关。
本研究表明,外阴 LP 和 LS 患者的性功能、生殖器自我形象和焦虑水平存在相关性。生殖器自我形象较差的 LP 和 LS 患者存在更多的性功能问题和焦虑状况。
