Different backgrounds may be involved in the elicitation of IgE-dependent reactions; an atopic background, i.e. synthesis of specific IgE against common allergens and an allergic background, in the field of drug allergy and allergy to hymenoptera stings, in which atopy is probably not an important risk factor. Allergens have three-dimensional molecular configurations and this is an important determinant of their immunological reactivity. Only multivalent allergens can induce a specific release of mediators.
