Barium granuloma (BG) is an unusual complication of barium enema that occurs due to the intravasation of barium into the colonic wall as a result of mucosal injury from an enema tip, overinflation of the rectal balloon, or intrinsic disease such as ulcerative colitis. We report a case of BG from a barium enema performed within 24 h after a proctoscopic biopsy. It was detected 5 years later when the patient developed persistent low back pain.
