Abstract

PURPOSE

The present study aimed to explore the value of DCE-MRI to evaluate the early efficacy of CyberKnife stereotactic radiosurgery in patients with symptomatic vertebral hemangioma (SVH).

METHODS

A retrospective analysis of patients with spinal SVH who underwent CyberKnife stereotactic radiosurgery from January 2017 to August 2019 was performed. All patients underwent DCE-MRI before treatment and three months after treatment. The parameters included volume transfer constant (K), transfer rate constant (K), and extravascular extracellular space volume fraction (V).

RESULTS

A total of 11 patients (11 lesions) were included. After treatment, six patients (54.5%) had a partial response, five patients (45.4%) had stable disease, and three patients (27.3%) presented with reossification. K and K decreased significantly in the third month after treatment (p = 0.003 and p = 0.026, respectively). ΔK was -46.23% (range, -87.37 to -23.78%), and ΔK was -36.18% (range, -85.62 to 94.40%). The change in V was not statistically significant (p = 0.213), and ΔV was -28.01% (range, -58.24 to 54.76%).

CONCLUSION

DCE-MRI parameters K and K change significantly after CyberKnife stereotactic radiosurgery for SVH. Thus, DCE-MRI may be of value in determining the early efficacy of CyberKnife stereotactic radiosurgery.

目的

本研究旨在探讨 DCE-MRI 评估 CyberKnife 立体定向放射外科治疗有症状性椎体血管瘤(SVH)患者早期疗效的价值。

方法

回顾性分析 2017 年 1 月至 2019 年 8 月接受 CyberKnife 立体定向放射外科治疗的脊柱 SVH 患者。所有患者均在治疗前和治疗后 3 个月行 DCE-MRI 检查。参数包括容积转移常数(K)、转移速率常数(K)和血管外细胞外间隙容积分数(V)。

结果

共纳入 11 例(11 个病灶)患者。治疗后,6 例(54.5%)患者有部分缓解,5 例(45.4%)患者病情稳定,3 例(27.3%)患者出现再骨化。治疗后 3 个月,K 和 K 显著降低(p=0.003 和 p=0.026)。ΔK 为-46.23%(范围:-87.37 至-23.78%),ΔK 为-36.18%(范围:-85.62 至 94.40%)。V 的变化无统计学意义(p=0.213),ΔV 为-28.01%(范围:-58.24 至 54.76%)。

结论
