Hydrogen peroxide is an antiseptic solution still often used in surgical departments for lavage of wounds. Its use is nevertheless linked to an important risk of gas embolism. Such a case report has not yet been published in the Czech literature and awareness of this danger is low among surgeons.
The author describes the case of gas embolism in a 40 years old patient after lavage of a periproctal abscess incision with 3% hydrogen peroxide. The lavage resulted in a cardiopulmonary arrest with the need of cardiopulmonary resuscitation. Fortunately, the patient recovered without any health consequences.
In the view of important risks and questionable and insufficiently proven benefits it may be the time to possibly reconsider the use of hydrogen peroxide in surgery and replace it with a different antiseptic agent.
过氧化氢是一种抗菌溶液,仍常在外科部门用于冲洗伤口。然而,其使用与气体栓塞的重大风险相关。这种情况下的报告在捷克文献中尚未发表,外科医生对这种危险的认识也很低。
作者描述了一位 40 岁患者在肛周脓肿切开术后用 3%过氧化氢冲洗时发生气体栓塞的病例。冲洗导致心肺骤停,需要心肺复苏。幸运的是,患者康复,没有任何健康后果。
鉴于重要的风险和可疑且未充分证明的益处,可能是时候重新考虑在外科手术中使用过氧化氢,并将其替换为其他抗菌剂。
