To explore the correlations of different appearances of labyrinthine 3D-FLAIR MRI with clinical features and prognosis in patients with idiopathic sudden sensorineural hearing loss (ISSNHL). Clinical data of patients with unilateral ISSNHL hospitalized from May 2017 to January 2019 were retrospectively analyzed. According to the results of 3D-FLAIR MRI, the patients were divided into three groups including hyperintense with absorption, hyperintense without absorption and normal. The differences and correlations among the three groups in clinical characteristics (gender, age, deafness side, duration, treatment days, dizziness/vertigo, basic diseases, vestibular function, deafness classification and typing) and prognosis were analyzed by SPSS 20.0 software. Data were collected from 1 245 cases, including 739 (59.36%) with normal signal, 288 (23.13%) hyperintense without absorption, and 218 (17.51%) hyperintense with absorption. The side ratio, treatment days, dizziness/vertigo incidence, vestibular dysfunction, deafness classification and typing were different among the three groups (<0.001). The incidence of right side was significantly higher in both the hyperintense with and without absorption groups than that in the normal. The vestibular dysfunction was more common in the hyperintense with absorption group than in the normal and hyperintense without absorption groups. It showed statistical differences in the dizziness/vertigo incidence, deafness classification, treatment days, and deafness typing compared between groups, which was the most significant in the hyperintense with absorption group, followed by the hyperintense without absorption group. There was no statistical difference in the total effective rate among the three groups (=0.139), whereas a significant difference in the recovery rate (<0.001). The prognosis was significantly correlated with duration, age, treatment days and dizziness/vertigo in the normal group (all <0.001), correlated with duration and treatment days in the hyperintense with absorption group (both <0.001), only correlated with the duration in the hyperintense without absorption group (<0.001). 3D-FLAIR MRI manifestation is closely related to the clinical features and efficacy of ISSNHL. It is helpful to clarify the pathology of inner ear, which is expected to be a new imaging indicator for disease evaluation.
探讨特发性突发性感音神经性听力损失(ISSNHL)患者迷路3D-FLAIR MRI不同表现与临床特征及预后的相关性。回顾性分析2017年5月至2019年1月住院的单侧ISSNHL患者的临床资料。根据3D-FLAIR MRI结果,将患者分为吸收性高信号、非吸收性高信号和正常三组。采用SPSS 20.0软件分析三组在临床特征(性别、年龄、耳聋侧别、病程、治疗天数、头晕/眩晕、基础疾病、前庭功能、耳聋分类及分型)及预后方面的差异和相关性。收集了1245例患者的数据,其中信号正常者739例(59.36%),非吸收性高信号者288例(23.13%),吸收性高信号者218例(17.51%)。三组的侧别比例、治疗天数、头晕/眩晕发生率、前庭功能障碍、耳聋分类及分型存在差异(<0.001)。吸收性和非吸收性高信号组的右侧发生率均显著高于正常组。吸收性高信号组的前庭功能障碍比正常组和非吸收性高信号组更常见。组间比较在头晕/眩晕发生率、耳聋分类、治疗天数及耳聋分型方面存在统计学差异,吸收性高信号组差异最显著,其次是非吸收性高信号组。三组的总有效率无统计学差异(=0.139),而治愈率有显著差异(<0.001)。正常组的预后与病程、年龄、治疗天数及头晕/眩晕显著相关(均<0.001),吸收性高信号组与病程和治疗天数相关(均<0.001),非吸收性高信号组仅与病程相关(<0.001)。3D-FLAIR MRI表现与ISSNHL的临床特征及疗效密切相关。有助于阐明内耳病理,有望成为疾病评估的新影像学指标。
