Neuralgia due to pressure on the inferior alveolar nerve by an impacted third molar is not uncommon but the entrapment of the nerve within the substance of the tooth is rare. When such a case is encountered, the removal of the tooth is indicated to relieve pain. The removal of the tooth usually results in a degree of impairment of sensation in the area supplied by the nerve, for a period of time. However, if care is taken to preserve the nerve during surgery, the period of impaired sensation can be shortened. An impacted tooth involving the inferior alveolar nerve but not producing symptoms should not be removed.
