Abstract

OBJECTIVE

This study was designed to investigate the sensitivity and specificity of the three-dimensional (3D) pelvic floor ultrasound and the conventional ultrasound in pelvic floor assessment after delivery.

METHODS

A total of 108 patients with pelvic floor dysfunction treated in our hospital were selected as the study group, and another 108 healthy subjects were included as the control group. Both groups were diagnosed by conventional ultrasound and 3D pelvic floor ultrasound, and confirmed with pathological findings. The diagnostic results of the conventional ultrasound and 3D pelvic floor ultrasound were compared, and the diagnostic conditions of various indicators in the two groups were compared.

RESULTS

The numbers of true positive cases and true negative cases diagnosed by the conventional ultrasound were 84 and 90, respectively, while the numbers of true positive cases and true negative cases diagnosed by the 3D pelvic floor ultrasound were 8 and 102, respectively (<0.05). The accuracy, specificity, and sensitivity of the conventional ultrasound were 80.6%, 83.3%, and 77.8%, respectively, which were lower than 94.4%, 96.3%, and 92.6% of the 3D pelvic floor ultrasound, respectively (<0.05). The study group exhibited greater urethral rotation angle (URA), posterior urethrovesical angles (PUVA) under Vsalva maneuver, area of the pelvic hiatus (APH), cystoscopic descent distance, cervical descent distance, and PUVA at resting state (<0.05), and smaller angle of urethral inclination (UI), anal levator-urethra gap (LUG), cervix-symphyseal distance (CSD), rectal ampulla-symphyseal distance (RSD) and bladder neck-symphysis pubis distance (BSD) compared with the control group (<0.05).

CONCLUSION

The 3D pelvic floor ultrasound is more accurate, specific and sensitive than the conventional ultrasound in the assessment of postpartum pelvic floor function, which is worthy of wide application.

目的

本研究旨在探讨三维(3D)盆底超声和传统超声在产后盆底评估中的敏感性和特异性。

方法

选取我院收治的108例盆底功能障碍患者作为研究组,另选108例健康受试者作为对照组。两组均采用传统超声和3D盆底超声进行诊断,并经病理结果证实。比较传统超声和3D盆底超声的诊断结果,以及两组各项指标的诊断情况。

结果

传统超声诊断的真阳性病例数和真阴性病例数分别为84例和90例,而3D盆底超声诊断的真阳性病例数和真阴性病例数分别为8例和102例(<0.05)。传统超声的准确性、特异性和敏感性分别为80.6%、83.3%和77.8%,分别低于3D盆底超声的94.4%、96.3%和92.6%(<0.05)。研究组在Valsalva动作下的尿道旋转角(URA)、后尿道膀胱角(PUVA)、骨盆裂孔面积(APH)、膀胱镜下下降距离、宫颈下降距离和静息状态下的PUVA均较大(<0.05),而尿道倾斜角(UI)、肛门提肌-尿道间隙(LUG)、宫颈-耻骨联合距离(CSD)、直肠壶腹-耻骨联合距离(RSD)和膀胱颈-耻骨联合距离(BSD)均小于对照组(<0.05)。

结论

3D盆底超声在产后盆底功能评估中比传统超声更准确、特异和敏感,值得广泛应用。
