Spinal subdural abscesses (SSAs) are rare pathologies presenting as encapsulated pus located intradurally and extramedullary. Although there is no uniform opinion on the cause of this pathology, approximately 50% of cases are attributed to infection. Here, we present a rare case of a female patient who presented to N.N. Burdenko Neurosurgical Center for treatment of an extradural tumor in the lower lumbar spine. She complained of acute lower back pain, lower limb muscle spasms, progressive lower limb weakness, numbness in toes, and increased frequency of defecation (five to six times per day). Intraoperatively, we discovered that the epidural space was clear and a subdural abscess was located and removed. The patient was started on antibiotics and recovered 29 days later. This case report illustrates an atypical SSA mimicking an extramedullary tumor on MRI.
脊髓硬膜下脓肿（SSA）是一种罕见的病理状况，表现为硬膜内和髓外的包裹性脓液。尽管对于这种病理状况的病因尚无统一观点，但约50%的病例归因于感染。在此，我们报告一例罕见病例，一名女性患者因下腰椎硬膜外肿瘤前往N.N. 布尔坚科神经外科中心接受治疗。她主诉急性下背部疼痛、下肢肌肉痉挛、进行性下肢无力、脚趾麻木以及排便频率增加（每天五到六次）。术中，我们发现硬膜外间隙清晰，定位并切除了一个硬膜下脓肿。患者开始使用抗生素治疗，29天后康复。本病例报告说明了一例在MRI上表现为类似髓外肿瘤的非典型SSA。
