Abstract

BACKGROUND AND AIMS

To compare the effects of real-time and retrospective flash glucose monitoring (FGM) on daily glycemic control and lifestyle in patients with type 2 diabetes on premix insulin therapy.

METHODS AND RESULTS

A total of 172 patients using premix insulin, with HbA1c ≥ 7.0% (56 mmol/mol), or the time below the target (TBR) ≥ 4%, or the coefficient of variation (CV) ≥36% during the screening period, were randomly assigned to retrospective FGM (n = 89) or real-time FGM group (n = 83). Another two retrospective or real-time 14-day FGMs were performed respectively, 1 month apart. Both groups received educations and medication adjustment after each FGM. Time in range (3.9~10.0 mmol/l, TIR) increased significantly after 3 months in the real-time FGM group (6.5%) compared with the retrospective FGM group (-1.1%) ( = 0.014). HbA1c decreased in both groups (both < 0.01). Real-time FGMs increased daily exercise time compared with the retrospective group ( = 0.002).

CONCLUSIONS

Real-time FGM with visible blood glucose improves daily glycemic control and diabetes self-care behaviors better than retrospective FGM in patients with type 2 diabetes on premix insulin therapy.

背景与目的

比较实时和回顾性动态血糖监测(FGM)对接受预混胰岛素治疗的 2 型糖尿病患者日常血糖控制和生活方式的影响。

方法与结果

共有 172 名使用预混胰岛素的患者,其糖化血红蛋白(HbA1c)≥7.0%(56mmol/mol),或目标时间范围内(TBR)<4%,或变异系数(CV)≥36%,在筛选期接受回顾性 FGM(n=89)或实时 FGM 组(n=83)。两组患者分别进行了另外两次回顾性或实时 14 天 FGM 检查,相隔 1 个月。每组在每次 FGM 后都接受了教育和药物调整。与回顾性 FGM 组(-1.1%)相比,实时 FGM 组在 3 个月时 TIR(3.9~10.0mmol/l)显著增加(6.5%)(=0.014)。两组的 HbA1c 均下降(均<0.01)。与回顾性组相比,实时 FGM 组增加了每日运动时间(=0.002)。

结论

与回顾性 FGM 相比,接受预混胰岛素治疗的 2 型糖尿病患者中,实时 FGM 可改善日常血糖控制和糖尿病自我护理行为。
