Abstract

BACKGROUND

Pancreatic adenocarcinoma is one of the most common malignant tumors of the digestive system. More than 80% of patients with pancreatic adenocarcinoma are not diagnosed until late stage and have distant or local metastases.

AIM

To investigate the value of computed tomography (CT) perfusion imaging in the evaluation of angiogenesis in pancreatic adenocarcinoma patients.

METHODS

This is a retrospective cohort study. Patients with pancreatic adenocarcinoma and volunteers without pancreatic diseases underwent CT perfusion imaging from December 2014 to August 2017 in Huashan Hospital, Fudan University Shanghai, China.

RESULTS

A total number of 35 pancreatic adenocarcinoma patients and 33 volunteers were enrolled. The relative blood flow (rBF), and relative blood volume (rBV) were significantly lower in patients with pancreatic adenocarcinoma than in the control group ( 0.05). Conversely, the relative permeability in patients with pancreatic adenocarcinoma was significantly higher than that in controls ( 0.05). In addition, rBF, rBV, and the vascular maturity index (VMI) were significantly lower in grade III-IV pancreatic adenocarcinoma than in grade I-II pancreatic adenocarcinoma ( 0.05). Vascular endothelial growth factor (VEGF), CD105-MVD, CD34-MVD, and angiogenesis rate (AR) were significantly higher in grade III-IV pancreatic adenocarcinoma than in grade I-II pancreatic adenocarcinoma ( 0.05). Significant correlations between rBF and VEGF, CD105-MVD, AR, and VMI ( 0.01) were observed. Moreover, the levels of rBV were statistically significantly correlated with those of VEGF, CD105-MVD, CD34-MVD, and VMI ( 0.01).

CONCLUSION

Perfusion CT imaging may be an appropriate approach for quantitative assessment of tumor angiogenesis in pancreatic adenocarcinoma.

背景

胰腺腺癌是消化系统最常见的恶性肿瘤之一。超过80%的胰腺腺癌患者直到晚期才被诊断出来,并且已经发生远处或局部转移。

目的

探讨计算机断层扫描(CT)灌注成像在评估胰腺腺癌患者血管生成中的价值。

方法

这是一项回顾性队列研究。2014年12月至2017年8月,在中国上海复旦大学附属华山医院,对胰腺腺癌患者和无胰腺疾病的志愿者进行了CT灌注成像检查。

结果

共纳入35例胰腺腺癌患者和33名志愿者。胰腺腺癌患者的相对血流量(rBF)和相对血容量(rBV)显著低于对照组(P<0.05)。相反,胰腺腺癌患者的相对通透性显著高于对照组(P<0.05)。此外,III-IV级胰腺腺癌的rBF、rBV和血管成熟指数(VMI)显著低于I-II级胰腺腺癌(P<0.05)。III-IV级胰腺腺癌的血管内皮生长因子(VEGF)、CD105标记的微血管密度(CD105-MVD)、CD34标记的微血管密度(CD34-MVD)和血管生成率(AR)显著高于I-II级胰腺腺癌(P<0.05)。观察到rBF与VEGF、CD105-MVD、AR和VMI之间存在显著相关性(P<0.01)。此外,rBV水平与VEGF、CD105-MVD、CD34-MVD和VMI水平在统计学上显著相关(P<0.01)。

结论

CT灌注成像可能是定量评估胰腺腺癌肿瘤血管生成的一种合适方法。
