The aim of this study was to investigate the feasibility of transferring the free flow-through dorsoulnar perforator flap to reconstruct severely injured fingers. From January 2015 to March 2019, 16 patients, 16 thumb or fingers volar partial defects concomitant with segmental digital arterial defects, were included in this study. The flow-through dorsoulnar perforator flaps were used to repair the finger wounds and reconstruct digit arteries. Sixteen fingers in 16 patients were successfully reconstructed and all flaps survived completely. At the final follow-up, the appearance of the affected fingers was aesthetically pleasing. Two-point discrimination of the fingertips was 7.5-11.0 mm. According to the Total Active Motion (TAM) method: nine fingers were excellent, four were good, and three were fair. The free flow-through dorsoulnar perforator flap can achieve the goal of mending finger soft-tissue conditions complicated with segmental digital arterial defects with good results. 本研究旨在探讨游离穿支皮瓣向掌侧转移修复严重手指损伤的可行性。2015 年 1 月至 2019 年 3 月,16 例拇指或手指掌侧部分缺损伴节段性指动脉缺损的患者纳入本研究。采用游离穿支皮瓣修复手指创面,重建指动脉。16 例患者的 16 个手指均成功重建,所有皮瓣完全存活。最终随访时,患指外观美观。指尖两点辨别觉为 7.5-11.0mm。根据总主动活动度(TAM)方法:9 个手指为优,4 个手指为良,3 个手指为可。游离穿支皮瓣可达到修复伴有节段性指动脉缺损的手指软组织条件的目的,效果良好。关键词:穿支皮瓣;皮瓣;趾背动脉;软组织缺损。
