Failed back surgery syndrome (FBSS) is a highly prevalent condition in patients after spine surgery. Although magnetic resonance imaging (MRI) is the gold standard for the diagnosis of epidural fibrosis, it is sometimes difficult to determine if epidural fibrosis contributes to radiculopathy. Herein, we share our experience in locating radiculopathy lesions using simultaneous positron emission tomography (PET)/MRI. 2-[F]-FDG (F-fluorodeoxyglucose) simultaneous PET/MRI maps of body glucose metabolism detected using PET can be used to correlate anatomical details provided by MRI to offer a very clear picture of neural inflammation due to extensive epidural fibrosis. More applications of 2-[F]-FDG simultaneous PET/MRI in low back pain and other musculoskeletal diseases should be further investigated in the future.
腰椎手术失败综合征(FBSS)在脊柱手术后的患者中是一种非常普遍的病症。尽管磁共振成像(MRI)是诊断硬膜外纤维化的金标准,但有时很难确定硬膜外纤维化是否导致神经根病。在此,我们分享我们使用同步正电子发射断层扫描(PET)/MRI定位神经根病病变的经验。利用PET检测的2-[F]-FDG(氟脱氧葡萄糖)同步PET/MRI人体葡萄糖代谢图,可将MRI提供的解剖细节关联起来,以清晰呈现因广泛硬膜外纤维化导致的神经炎症。未来应进一步研究2-[F]-FDG同步PET/MRI在腰痛和其他肌肉骨骼疾病中的更多应用。
