Foreign body-induced perforations of the ileum are rare consequences among adults.
This is a case report of a delayed presentation of an ileal perforation and concurrent faecal peritonitis presented as an acute abdomen, resultant from an ingested fishbone, which led to fatality despite urgent laparotomy.
Perforations following ingested foreign bodies are frequently unanticipated clinically, and diagnosed during advanced imaging studies or surgical interventions. Endoscopy, laparoscopy, and laparotomy have been used during surgical management in reported cases; however, prior early identification is pivotal for good outcomes as delayed presentations and delayed diagnosis carry a poorer prognosis.
Despite fish bones being frequent foreign bodies in the gastrointestinal tract and the majority causing no life-threatening adverse effects, they are the leading foreign bodies instigating gastrointestinal perforations. A high degree of suspicion is required when attending to patients with suspected gastrointestinal perforations and absent typical findings in routine imaging, where fish bones could be the aetiology.

吞食异物后的穿孔在临床上常常难以预料,多在进行高级影像学检查或手术干预时才被诊断出来。在已报道的病例中,手术治疗时曾使用过内镜检查、腹腔镜检查和剖腹手术;然而,早期识别对于良好预后至关重要,因为延迟就诊和延迟诊断的预后较差。
尽管鱼骨是胃肠道中常见的异物,且大多数不会造成危及生命的不良反应,但它们却是导致胃肠道穿孔的主要异物。在诊治疑似胃肠道穿孔且常规影像学检查无典型表现的患者时,应高度怀疑鱼骨可能是病因。
