To investigate the effects of lymphocyte (LY), C-reactive protein (CRP) and prealbumin (PA) levels on the clinical typing and course of disease in children infected with novel coronavirus (2019-nCoV) at the early stage.
A total of 140 children with 2019-nCoV infection diagnosed in Shijiazhuang People's Hospital and Hebei Provincial Chest Hospital from January 2021 to February 2021 were selected for this study. According to the clinical symptoms, laboratory results and imaging examination, the children were divided into asymptomatic infection group, mild infection group and common infection group. The levels of white blood cell (WBC), LY, CRP, PA, albumin (ALB), aspartate aminotransferase (AST), alanine aminotransferase (ALT), creatine kinase (CK) and creatine kinase MB isoenzyme (CKMB) in the children were recorded on the 2nd d after the positive detection of 2019-nCoV nucleic acid.
There were 73(52.1%) children in the asymptomatic infection group, 35(25.0%) children in the mild infection group and 32(22.9%) children in the common infection group. LY level in the common infection group was lower than that in the asymptomatic infection group and the mild infection group (F= 3.152, both < 0.05). CRP level in the common infection group was higher than that in the asymptomatic infection group and the mild infection group (F= 6.343, both 0.05). CRP level in the mild infection group was higher than that in the asymptomatic infection group (t= 2.052, 0.05). PA level in the common infection group and the mild infection group was lower compared with the asymptomatic infection group (F= 5.229, both 0.05). WBC, ALB, AST, ALT, CK and CKMB levels in the three groups showed no statistical significance (F= 1.803, F= 1.208, F= 2.391, F= 1.973, F= 0.401, F= 1.332, respectively, all 0.05). Correlation analysis demonstrated that LY and PA levels were negatively correlated with hospital stay (r= -0.265, r= -0.325, both 0.050), but CRP level was not correlated with hospital stay (r= -0.039, 0.05).
CRP is correlated with the clinical typing of children with 2019-nCoV infection, while LY and PA levels may be closely correlated with the clinical typing and course of treatment of children with 2019-nCoV infection.
探讨淋巴细胞(LY)、C反应蛋白(CRP)及前白蛋白(PA)水平对新型冠状病毒(2019 - nCoV)感染儿童早期临床分型及病程的影响。
选取2021年1月至2021年2月在石家庄市人民医院和河北省胸科医院确诊的140例2019 - nCoV感染儿童作为研究对象。根据临床症状、实验室检查结果及影像学检查,将患儿分为无症状感染组、轻症感染组和普通感染组。记录患儿2019 - nCoV核酸检测阳性后第2天的白细胞(WBC)、LY、CRP、PA、白蛋白(ALB)、天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、肌酸激酶(CK)及肌酸激酶同工酶MB(CKMB)水平。
无症状感染组73例(52.1%),轻症感染组35例(25.0%),普通感染组32例(22.9%)。普通感染组LY水平低于无症状感染组和轻症感染组(F = 3.152,均P < 0.05)。普通感染组CRP水平高于无症状感染组和轻症感染组(F = 6.343,均P < 0.05)。轻症感染组CRP水平高于无症状感染组(t = 2.052,P < 0.05)。普通感染组和轻症感染组PA水平低于无症状感染组(F = 5.229,均P < 0.05)。三组WBC、ALB、AST、ALT、CK及CKMB水平差异无统计学意义(F分别为1.803、1.208、2.391、1.973、0.401、1.332,均P > 0.05)。相关性分析显示,LY和PA水平与住院天数呈负相关(r分别为 - 0.265、 - 0.325,均P < 0.050),但CRP水平与住院天数无相关性(r = - 0.039,P > 0.05)。
CRP与2019 - nCoV感染儿童的临床分型相关,而LY和PA水平可能与2019 - nCoV感染儿童的临床分型及治疗病程密切相关。
