Abstract

INTRODUCTION

Due to the COVID-19 pandemic, in-person psychiatric care decreased, but mental disorders, such as post-traumatic stress disorder, anxiety, depression, obsessive compulsive symptoms and insomnia increased. Our objective was to describe the usefulness, advantages, disadvantages and limitations of telemedicine used in psychiatric patients' clinical care before and during the COVID-19 pandemic.

METHODS

A critical search was carried out on platforms such as PubMed, Cochrane, Lilacs and academic Google, in addition to an in-depth assessment of all the primary studies that answer our questions.

RESULTS

Telepsychiatry is a tool that can help resolve the need for psychiatric care in a more comprehensive way. Advantages include cost reduction, enabling care provision to rural areas, the fact that its effectiveness is comparable to in-person care and its use is suitable in a variety of scenarios, and also that it helps reduce stigma and enables continuous training processes among medical staff. In the context of the current pandemic, it reduces the risk of transmission by maintaining biosecurity measures. However, there are also limitations in its use, such as the collection of medical insurance payments, mandatory in-person visits for examinations or procedures, difficulty in developing a doctor-patient relationship, and uncertainty about how to give informed consent and maintain the patient's privacy. Additionally, there is the difficulty in controlling the patient's environment, with the risk of the consultation taking place somewhere inappropriate, and the fact that there are certain populations which would not benefit from the use of telepsychiatry and areas where its use is made difficult by a greater digital gap. Based on these advantages and disadvantages, recommendations can be made when considering telepsychiatry as a medical care option.

由于新冠疫情,面对面的精神科护理减少,但创伤后应激障碍、焦虑、抑郁、强迫症状和失眠等精神障碍有所增加。我们的目的是描述在新冠疫情之前和期间,远程医疗在精神科患者临床护理中的实用性、优点、缺点和局限性。

方法

除了对所有回答我们问题的主要研究进行深入评估外,还在PubMed、Cochrane、Lilacs和学术谷歌等平台上进行了严格的检索。

结果

远程精神病学是一种有助于更全面地解决精神科护理需求的工具。优点包括降低成本、能够为农村地区提供护理、其有效性与面对面护理相当且适用于多种场景,还能帮助减少污名化并促进医护人员的持续培训。在当前疫情背景下,它通过维持生物安全措施降低了传播风险。然而,其使用也存在局限性,如医疗保险支付的收取、检查或程序必须进行面对面就诊、难以建立医患关系、在如何获得知情同意和维护患者隐私方面存在不确定性。此外,难以控制患者的环境,存在会诊在不合适地点进行的风险,而且某些人群无法从远程精神病学中受益,以及在数字鸿沟较大的地区使用存在困难。基于这些优缺点,在将远程精神病学作为医疗护理选择时可以提出建议。
