Abstract

BACKGROUND

Given the many complications of drug therapy, it seems reasonable to use non-pharmacological therapies that can improve mental and physical disorders in haemodialysis patients.

AIMS

This study aims to determine the effectiveness of cognitive behavioural group therapy for insomnia (CBGT-I) in sleep quality, depression, anxiety and general psychological health of haemodialysis patients.

METHOD

This randomized clinical trial was conducted on 116 haemodialysis patients who were randomly assigned to experimental (=58) and control (=58) groups. In the experimental group, CBGT-I was provided during nine weekly sessions. Data collection tools included Pittsburgh Sleep Quality Index (PSQI), Beck Depression Inventory-II (BDIII), Beck Anxiety Inventory (BAI), General Health Questionnaire (GHQ-28), Clinical Global Improvement Scale (CGI), Client Satisfaction Questionnaire (CSQ) and Working Alliance Inventory-Short Form (WAI-S). Data were analysed by SPSS-25 and <.05 was considered significant.

RESULTS

The findings demonstrated that CBGT-I compared with control group was effective in improving sleep quality (<.001, η=.790), depression (<.001, η=.616), anxiety (<.001, η=.682) and general psychological health (<.001, η=.871). Participants of CBGT-I showed notable improvements as a result of the treatment, were satisfied with treatment, and had a good therapeutic relationship.

CONCLUSIONS

CBGT-I is effective in reducing depression and anxiety in addition to improving sleep quality and general psychological health in haemodialysis patients. Therefore, it is recommended to be used as a complementary treatment for these patients.

鉴于药物治疗存在诸多并发症,使用非药物疗法来改善血液透析患者的精神和身体障碍似乎是合理的。

目的

本研究旨在确定认知行为团体治疗失眠症(CBGT-I)对血液透析患者睡眠质量、抑郁、焦虑和一般心理健康的疗效。

方法

这是一项随机临床试验,共纳入 116 名血液透析患者,随机分为实验组(n=58)和对照组(n=58)。实验组接受为期 9 周的 CBGT-I 治疗。研究工具包括匹兹堡睡眠质量指数(PSQI)、贝克抑郁量表-II(BDIII)、贝克焦虑量表(BAI)、一般健康问卷(GHQ-28)、临床总体印象量表(CGI)、患者满意度问卷(CSQ)和工作联盟量表-短式(WAI-S)。数据分析采用 SPSS-25 软件,P 值<.05 为差异有统计学意义。

结果

研究结果表明,与对照组相比,CBGT-I 可有效改善睡眠质量(<.001,η=.790)、抑郁(<.001,η=.616)、焦虑(<.001,η=.682)和一般心理健康(<.001,η=.871)。CBGT-I 组的患者在治疗后表现出显著的改善,对治疗满意,并且与治疗师建立了良好的治疗关系。

结论

CBGT-I 除了能改善血液透析患者的睡眠质量和一般心理健康外,还能有效减轻抑郁和焦虑。因此,建议将其作为这些患者的辅助治疗方法。
