Until now pathogenesis and the choice of surgical procedure in the management of gastric ulcer has suffered due to lack of resolution. The pathogenetic value of duodenogastric reflux for example has been underestimated and has not been considered in the operative technique. In order to close this gap the primary of secondary isoperistaltic orthograde interpositon of a jejunal loop between gastric remnant and duodenum is recommended.
