Morel-Lavalleé lesion is an uncommon closed degloving injury usually affecting the lower extremity. Although these lesions have been documented in literature, yet there is no standard treatment algorithm for the same. A case of Morel-Lavallée lesion following blunt injury to the thigh is therefore presented to highlight the diagnostic and therapeutic challenges in managing such lesions. The aim of presenting the case is to create awareness of clinical presentation, diagnosis, and management of Morel-Lavallée lesions, especially in the setting of polytrauma patients.
A case of Morel-Lavallée lesion in a 32-year-old male with history of a blunt injury to the right thigh caused by a partial run over accident is presented. A magnetic resonance imaging (MRI) was done to confirm the diagnosis. A limited open approach for evacuating the fluid in the lesion was performed followed by irrigation of the cavity with a combination of 3% hypertonic saline and hydrogen peroxide in order to induce fibrosis to obliterate the dead space. This was followed by continuous negative suction accompanied with a pressure bandage.
A high index of suspicion is necessary especially in cases of severe blunt injuries to the extremities. MRI is essential for early diagnosis of Morel-Lavallée lesions. A limited open approach is a safe and effective option for treatment. The use of 3% hypertonic saline along with hydrogen peroxide irrigation of the cavity to induce sclerosis is a novel method for treating the condition.
莫雷尔-拉瓦利埃损伤是一种罕见的闭合性脱套伤,通常累及下肢。尽管这些损伤在文献中有记载,但对于其治疗尚无标准的算法。因此,本文报告一例大腿钝器伤后出现莫雷尔-拉瓦利埃损伤的病例,以突出此类损伤在诊断和治疗方面的挑战。展示该病例的目的是提高对莫雷尔-拉瓦利埃损伤临床表现、诊断和治疗的认识,尤其是在多发伤患者的情况下。
本文报告一例32岁男性,因部分碾压事故导致右大腿钝器伤后出现莫雷尔-拉瓦利埃损伤。进行了磁共振成像(MRI)以确诊。采用有限切开的方法清除损伤部位的液体,随后用3%高渗盐水和过氧化氢混合液冲洗腔隙,以诱导纤维化消除死腔。之后进行持续负压吸引并加压包扎。
尤其是在四肢严重钝器伤的情况下,必须保持高度怀疑。MRI对于莫雷尔-拉瓦利埃损伤的早期诊断至关重要。有限切开是一种安全有效的治疗选择。使用3%高渗盐水联合过氧化氢冲洗腔隙以诱导硬化是治疗该疾病的一种新方法。
