Myocardial infarction with nonobstructive coronary arteries (MINOCA) has poor long-term cardiovascular outcomes, similar to myocardial infarction with conventional atherogenic coronary artery disease. However, MINOCA-related mechanical complications are rarely reported. We report a case of an octogenarian woman diagnosed with MINOCA-related ventricular septal rupture assessed by multimodal images, including autopsy findings. ().
