Paraosteal lipomas are rare benign neoplasm composed of mature adipose tissue with underlying bone changes. They are extremely rare to occur in head and neck region. We present a case of a 40-year-old male with a paraosteal lipoma of occipital region. The diagnosis of the lesion was reached by clinical examination and confirmed on computed tomography (CT) and Magnetic resonance imaging (MRI).
