BACKGROUND
Cerebellopontine angle lipoma is a rare tumor that composes less than 1% of all CPA tumors. There has been no recorded case of unilateral CPA/IAC lipoma with sudden contralateral deafness yet.
CASE PRESENTATION
We report a 52-year-old man diagnosed with right cerebellopontine angle lipoma and combined left total deafness. Pure-tone audiometry revealed total sensorineural deafness in his left ear and moderate sensorineural deafness in the right ear. The patient was treated with glucocorticoids, batroxobin, and other symptomatic treatments. There was no substantial improvement in hearing after 14 days’ treatment.
DISCUSSION
We chose conservative treatment for him. It is advised to wear hearing aids in the right ear and to undergo regular imaging monitoring.
CONCLUSION
Treatment options for such patients should be chosen by taking into account the degree of bilateral hearing loss, the size and location of the tumor, the possibility of preserving hearing during surgery, the functional level of the patient's facial nerve, and other factors.
背景
桥小脑角脂肪瘤是一种罕见肿瘤，占所有桥小脑角肿瘤的比例不到1%。目前尚无单侧桥小脑角/内听道脂肪瘤伴突发对侧耳聋的病例报道。
病例介绍
我们报告一名52岁男性，被诊断为右侧桥小脑角脂肪瘤并伴有左侧全聋。纯音听力测试显示其左耳为全感音神经性耳聋，右耳为中度感音神经性耳聋。该患者接受了糖皮质激素、巴曲酶及其他对症治疗。治疗14天后听力无明显改善。
讨论
我们为他选择了保守治疗。建议右耳佩戴助听器并定期进行影像学监测。
结论
对于此类患者，应综合考虑双侧听力损失程度、肿瘤大小和位置、手术中保留听力的可能性、患者面神经功能水平等因素来选择治疗方案。
