A patient with acute posteroinferior myocardial infarction and second-degree type I AV block within the AV node are presented. After the onset of paroxysmal atrial fibrillation marked the slowing of the heart rate and major syncopal attack unexpectedly appeared necessitating temporary pacing. This case illustrates that temporary pacing may be necessary unexpectedly in some patients with acute myocardial infarction who did not meet rigidly the criteria for transvenous catheter insertion.
