Following vaccination with adenoviral vector-based ChAdOx1 nCoV-19, serious neurological adverse events have been reported. Here we report two cases who presented with quadriparesis following the adenoviral vector-based ChAdOx1 nCoV-19 vaccine. A 55-year-old male patient presented with quadriparesis after 8 days of the second dose of ChAdOx1 nCoV-19 vaccination. Imaging showed features of stroke with right basilar artery thrombosis; he was started on anticoagulation following which the patient's neurological status improved and he was discharged during the 7 week of hospital stay. A 19-year-old male patient presented with quadriparesis after 16 days of the first dose of ChAdOx1 nCoV-19 vaccination. Cerebral spinal fluid and nerve conduction study was suggestive of Guillain-Barre syndrome (GBS). Two doses of intravenous immunoglobulin were given, following which the patient's neurological status improved and he was discharged in the 11 week of his hospital stay. Awareness of neurological adverse effects and emphasis on the underlying mechanism of vaccine-induced thrombotic thrombocytopenia (VITT) and molecular mimicry in patients presenting with quadriparesis following ChAdOx1 nCoV-19 vaccination is important.
在接种基于腺病毒载体的ChAdOx1 nCoV-19疫苗后,已报告了严重的神经系统不良事件。在此,我们报告两例在接种基于腺病毒载体的ChAdOx1 nCoV-19疫苗后出现四肢瘫痪的病例。一名55岁男性患者在接种第二剂ChAdOx1 nCoV-19疫苗8天后出现四肢瘫痪。影像学检查显示有右侧基底动脉血栓形成的中风特征;开始给予抗凝治疗,之后患者的神经状态有所改善,并在住院第7周出院。一名19岁男性患者在接种第一剂ChAdOx1 nCoV-19疫苗16天后出现四肢瘫痪。脑脊液和神经传导研究提示为吉兰-巴雷综合征(GBS)。给予两剂静脉注射免疫球蛋白,之后患者的神经状态有所改善,并在住院第11周出院。认识到神经系统不良反应,并重视接种ChAdOx1 nCoV-19疫苗后出现四肢瘫痪的患者中疫苗诱导的血栓性血小板减少症(VITT)和分子模拟的潜在机制很重要。
