Automatic synthesis of F-AV-45 and its clinical application in Alzheimer's disease.

Abstract

OBJECTIVE

To investigate the automatic synthesis of β-amyloid (Aβ) positron emission tomography (PET) imaging agent (E) -4- (2- (6- (2- (2-F fluoroethoxy) ethoxy) ethoxy) pyridine-3-yl) vinyl) - N-methylaniline (F-AV-45) for the diagnosis of Alzheimer's disease (AD) and its clinical application in AD patients.

MATERIALS AND METHODS

Fluorine-18-AV-45 was synthesized with AV-105 as the precursor, and the factors affecting the synthesis efficiency, such as the amount of precursor, nucleophilic reaction temperature were studied. At the same time, F-AV-45 PET/computed tomography (CT) brain scanning was performed in 15 patients with dementia to determine whether AD was the cause of the dementia.

RESULTS

After optimizing the parameters, it was discovered that the highest synthesis efficiency was achieved with a AV-105 dosage of 2mg, a reaction temperature of 130C, and 1mL of DMSO. The radiochemical yield (RCP) was greater than 98, and the uncorrected synthesis efficiency was about 31.0%±2.8%. Ten of the 15 patients with dementia showed positive Aβ protein deposition, and the main deposition site of the imaging agent was the gray matter area of the brain, which was consistent with AD diagnosis, while the other 5 patients showed negative Aβ protein deposition, suggesting non-AD dementia.

CONCLUSION

β-amyloid protein F-AV-45 imaging agent can be easily and quickly prepared by the All in One radiochemical synthesis module. Our preliminary results offer hope that it can effectively detect β-amyloid deposition in the brain of AD patients in order to determine the etiology of dementia.

材料和方法

以 AV-105 为前体,合成氟-18-AV-45,研究影响合成效率的因素,如前体用量、亲核反应温度等。同时,对 15 例痴呆患者进行 F-AV-45 PET/CT 脑扫描,以确定痴呆是否由 AD 引起。

结果

优化参数后发现,AV-105 用量为 2mg、反应温度为 130°C、DMSO 用量为 1mL 时,合成效率最高。放射性化学产率(RCP)大于 98%,未经校正的合成效率约为 31.0%±2.8%。15 例痴呆患者中,有 10 例显示 Aβ 蛋白沉积阳性,成像剂的主要沉积部位为大脑灰质区,与 AD 诊断一致,而另外 5 例显示 Aβ 蛋白沉积阴性,提示非 AD 痴呆。

结论

通过 All in One 放射性化学合成模块,可方便、快速地制备β-淀粉样蛋白 F-AV-45 成像剂。我们的初步结果表明,该成像剂有望有效检测 AD 患者大脑中的β-淀粉样蛋白沉积,从而确定痴呆的病因。
