PURPOSE: The aim of this study was to compare the clinical outcomes between patients with chronic ankle instability (CAI) undergoing arthroscopic anterior talofibular ligament (ATFL) repair who received elastic bandage treatment and those who received lower-leg cast immobilization. METHODS: CAI patients with isolated ATFL injury undergoing arthroscopic ATFL repair from January 2017 and August 2019 were included in the study. The visual analogue scale (VAS) at rest and during activities, American Orthopedic Foot and Ankle Society (AOFAS) score, Karlsson Ankle Functional Score (Karlsson score), and time of returning to walk, walk normally, work and sports were evaluated preoperatively, and at 6 months and 12 months follow-up. RESULTS: A total of 41 patients were included in this study. Among them, 24 patients accepted lower-leg cast fixation, and the other 17 patients were immobilized with elastic bandage. Compared to patients with lower-leg immobilization, patients with elastic bandage fixation had significantly lower VAS during activities (P = 0.021) and higher AOFAS score (P = 0.015) at 12 months follow-up. The Karlsson score at 6 months follow-up were significantly higher in elastic bandage group than those in lower-leg group (P = 0.011). However, no significant difference was observed in time of returning to walk, work and sports between the two groups. CONCLUSION: Elastic bandage treatment was better than lower-leg cast immobilization in terms of eliminating pain symptom at 12 months follow-up, and improving ankle functional outcome at 6 months follow-up. Moreover, the present study emphasized that lower-leg cast immobilization offered no advantages in arthroscopic ATFL repair postoperative immobilization. 目的:本研究旨在比较慢性踝关节不稳(CAI)患者接受关节镜下距腓前韧带(ATFL)修复术后接受弹性绷带治疗与小腿石膏固定治疗的临床疗效。
方法:本研究纳入了 2017 年 1 月至 2019 年 8 月接受关节镜下 ATFL 修复术的孤立性 ATFL 损伤的 CAI 患者。术前、术后 6 个月和 12 个月评估静息和活动时的视觉模拟评分(VAS)、美国矫形足踝协会(AOFAS)评分、Karlsson 踝关节功能评分(Karlsson 评分)以及恢复行走、正常行走、工作和运动的时间。
结果:本研究共纳入 41 例患者,其中 24 例接受小腿石膏固定,17 例采用弹性绷带固定。与小腿固定组相比,弹性绷带固定组患者在术后 12 个月时活动时的 VAS 明显更低(P=0.021),AOFAS 评分明显更高(P=0.015)。弹性绷带组在术后 6 个月的 Karlsson 评分明显高于小腿组(P=0.011)。然而,两组患者在恢复行走、工作和运动的时间方面无显著差异。
结论:弹性绷带治疗在术后 12 个月时可更好地消除疼痛症状,在术后 6 个月时可更好地改善踝关节功能,优于小腿石膏固定。此外,本研究强调,在关节镜下 ATFL 修复术后固定中,小腿石膏固定并没有优势。
