An extended dorsalis pedis island flap was used to cover the heel in two patients. The arterial predicles were extended by cutting and bridging with vein grafts to allow the flaps to reach the defects. By this method, excellent coverage is obtained and the protective sensation may be retained or restored by preservation or extension of the sensory nerves.
