: The aim of the report is to report the outcomes of the medial rectus (MR) disinsertion procedure for the management of large-angle esotropia (ET) patients. : This is a retrospective case series of patients with large-angle ET who underwent an MR disinsertion procedure between March 2012 to April 2022. The procedure happened accidentally during muscle surgery. The demographic and clinical data, including sex, age, visual acuity, pre- and postoperative angle of strabismus, duction limitations, results of intraoperative forced duction tests, and follow-up duration were collected from medical records. : Five patients were enrolled in this study. The mean age was 62.2 ± 9.8 years, and the mean follow-up was 24.8 ± 8.7 months. The ET at the primary position of gaze was 92.0 ± 17.9 prism diopters (PD) before MR disinsertion and 38.0 ± 29.5 PD after MR disinsertion only. Abduction deficiency was -4 before after MR disinsertion, which improved to -1 at the last follow-up. : The results of MR disinsertion were not as frustrating as anticipated. MR disinsertion may be considered in patients with large-angle sensory ET who refuse surgery on the opposite eye.

这是一项回顾性病例系列研究，纳入了 2012 年 3 月至 2022 年 4 月期间因意外在肌肉手术中进行了 MR 离断术的大角度 ET 患者。收集了患者的人口统计学和临床数据，包括性别、年龄、视力、斜视术前和术后角度、眼球运动受限情况、术中强制眼球运动测试的结果以及随访时间。
本研究共纳入 5 例患者，平均年龄为 62.2 ± 9.8 岁，平均随访时间为 24.8 ± 8.7 个月。MR 离断术前第一眼注视时 ET 为 92.0 ± 17.9 棱镜度（PD），MR 离断术后为 38.0 ± 29.5 PD。MR 离断术后外展不足为-4，最后一次随访时改善至-1。
MR 离断术的结果并不如预期的那样令人沮丧。对于拒绝对侧眼手术的大角度感觉性 ET 患者，可考虑行 MR 离断术。
