Abstract

BACKGROUND

This study aims to explore the relationship between Lower limb alignment parameters and the degree of Medial Meniscal Extrusion (MME) in patients with Knee Osteoarthritis (KOA), in hopes of providing new reference data for the prevention and treatment of KOA.

METHODS

A retrospective study design was employed, analyzing 623 KOA patients treated at our hospital from 2022 to 2023, with 307 patients' information collected according to inclusion and exclusion criteria. Patients were divided into mild and severe groups based on the degree of MME, and differences in Lower limb alignment parameters between the two groups were compared. Univariate analysis, Pearson correlation analysis, and multivariate stepwise regression analysis were used, along with Receiver operating characteristic(ROC) curve assessment to evaluate the predictive value of Lower limb alignment parameters on the degree of MME.

RESULTS

Univariate analysis showed significant correlations between Anatomic Mechanical Axis (AMA), Joint Line Convergence Angle (JLCA), mechanical Lateral Distal Femur Angle (mLDFA), Medial Proximal Tibia Angle (MPTA), and the degree of MME (P < 0.05). Pearson correlation analysis further confirmed the positive correlations between these parameters and the degree of MME. Multivariate stepwise regression analysis indicated that AMA, JLCA, and mLDFA are significant independent predictors of the severity of MME. ROC analysis suggested that the combination of AMA, JLCA, and mLDFA could better predict severe MME.

CONCLUSION

AMA, JLCA, and mLDFA are closely related to the degree of MME in KOA patients and can serve as important parameters for assessing the severity of MME, holding significant implications for the early prevention and treatment of KOA.

本研究旨在探讨膝关节骨性关节炎(KOA)患者下肢对线参数与内侧半月板突出(MME)程度的关系,以期为 KOA 的防治提供新的参考数据。

方法

采用回顾性研究设计,分析我院 2022 年至 2023 年收治的 623 例 KOA 患者的资料,根据纳入和排除标准收集 307 例患者的信息。根据 MME 程度将患者分为轻度和重度两组,比较两组下肢对线参数的差异。采用单因素分析、Pearson 相关分析、多元逐步回归分析及受试者工作特征(ROC)曲线评估下肢对线参数对 MME 程度的预测价值。

结果

单因素分析显示解剖机械轴(AMA)、关节线会聚角(JLCA)、机械外侧远端股骨角(mLDFA)、内侧胫骨近端角(MPTA)与 MME 程度均存在显著相关性(P<0.05)。Pearson 相关分析进一步证实了这些参数与 MME 程度的正相关关系。多元逐步回归分析表明,AMA、JLCA 和 mLDFA 是 MME 严重程度的独立显著预测因子。ROC 分析提示 AMA、JLCA 和 mLDFA 的组合能更好地预测重度 MME。

结论

AMA、JLCA 和 mLDFA 与 KOA 患者 MME 程度密切相关,可作为评估 MME 严重程度的重要参数,对 KOA 的早期预防和治疗具有重要意义。
