Abstract

INTRODUCTION

Superior cluneal nerve (SCS) entrapment neuropathy is an often overlooked cause of low back pain. Nerve block is usually applied to the sensitive point on the iliac crest where Tinel positivity is detected, with a mixture of a corticosteroid and a local anesthetic. Dextrose injection is one of the treatment methods. There are not enough studies comparing the effectiveness of dextrose and steroid injections. In this study, it was planned to compare these two methods.

MATERIALS AND METHODS

This retrospective study included 16 patients who underwent ultrasound-guided steroid injection and 17 patients who underwent ultrasound-guided dextrose injection with the diagnosis of SCS neuropathy at Elazığ Fethi Sekin City Hospital Polyclinics between 01.09.2023 and 28.03.2024. Patients were evaluated with visual analog scale (VAS) and Roland-Morris Disability Questionnaire (RMDQ) scores before and after the procedure at week 1 Results: While VAS and RMDQ scores were similar in both groups before the procedure, VAS and RMDQ scores at week 1 were significantly lower in the dextrose group.

CONCLUSION

Perineural dextrose injection appears to be a safe and effective alternative to steroid injections in the treatment of SCS neuropathy.

引言

臀上皮神经(SCS)卡压性神经病变是导致腰痛的一个常被忽视的原因。神经阻滞通常应用于在髂嵴上检测到Tinel征阳性的敏感点,使用皮质类固醇和局部麻醉剂的混合液。葡萄糖注射是治疗方法之一。比较葡萄糖注射和类固醇注射疗效的研究并不充分。在本研究中,计划对这两种方法进行比较。

材料与方法

这项回顾性研究纳入了2023年9月1日至2024年3月28日期间在埃拉泽菲提·塞金市医院综合门诊被诊断为SCS神经病变并接受超声引导下类固醇注射的16例患者以及接受超声引导下葡萄糖注射的17例患者。在第1周手术前后,使用视觉模拟量表(VAS)和罗兰-莫里斯残疾问卷(RMDQ)评分对患者进行评估。结果:术前两组的VAS和RMDQ评分相似,但葡萄糖组在第1周时的VAS和RMDQ评分显著更低。

结论

在治疗SCS神经病变方面,神经周围葡萄糖注射似乎是类固醇注射的一种安全有效的替代方法。
