To investigate the value and sensitivity of contrast-enhanced ultrasonography (CEUS) combined with fine-needle aspiration biopsy (FNAB) in the diagnosis of thyroid nodules. This study enrolled 120 patients with thyroid nodules who visited our hospital between July 2018 and January 2021. All patients underwent CEU and CEUS-guided FNAB. The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of CEUS-guided FNAB for the qualitative diagnosis of thyroid nodules were significantly higher compared with CEUS only (P < 0.05). The consistency in the diagnostic results was high between CEU-guided FNAB and surgical pathology and moderate between CEU and surgical pathology. The diagnostic coincidence rates of CEUS-guided FNAB were significantly higher than those of CEU only for papillary thyroid carcinoma, follicular carcinoma, undifferentiated carcinoma, and medullary carcinoma (P < 0.05). The diagnostic coincidence rates of CEUS-guided FNAB for stages I, II, and III thyroid cancer were significantly higher compared with CEU only (P < 0.05). Contrast-enhanced ultrasonography combined with FNAB improved the sensitivity and accuracy of making a qualitative diagnosis for thyroid nodules. This outcome will not only help to distinguish thyroid cancer from benign ones but can also assist in determining the pathological type and tumor stage of thyroid cancer, thereby presenting good diagnostic value.
探讨超声造影(CEUS)联合细针穿刺活检(FNAB)在甲状腺结节诊断中的价值和灵敏度。本研究纳入了 2018 年 7 月至 2021 年 1 月期间在我院就诊的 120 例甲状腺结节患者。所有患者均行 CEU 和 CEUS 引导下 FNAB。CEUS 引导下 FNAB 对甲状腺结节定性诊断的灵敏度、特异度、准确性、阳性预测值和阴性预测值均显著高于 CEUS 单独检查(P<0.05)。CEUS 引导下 FNAB 与手术病理的诊断结果一致性较高,CEUS 与手术病理的一致性为中度。CEUS 引导下 FNAB 对甲状腺乳头状癌、滤泡癌、未分化癌和髓样癌的诊断符合率明显高于 CEUS 单独检查(P<0.05)。CEUS 引导下 FNAB 对Ⅰ、Ⅱ、Ⅲ期甲状腺癌的诊断符合率明显高于 CEUS 单独检查(P<0.05)。超声造影联合 FNAB 提高了甲状腺结节定性诊断的灵敏度和准确性。这一结果不仅有助于区分甲状腺癌和良性肿瘤,还可以辅助确定甲状腺癌的病理类型和肿瘤分期,从而具有良好的诊断价值。
